I live in the USA and my basic rules of tipping are:

1. Tip on the gross bill, not the bill after I've used a coupon

2. Tip $2 for bills under $10

3. Tip 20% (mostly calculated in my head) for bills over $10

4. Around the holidays maybe tip more just because

Mostly it's nice because I almost never think very much about how much I should tip.
